Jobsite Delivery Acknowledgement

> Customer understands that the material will deliver via full-size tractor trailer, which will be unloaded via attached lift.

> Adequate space is available to receive delivery at listed delivery address for maneuvering truck and trailer without damaging property on site or truck and trailer.

> Customer understands there is the possibility of damage to driveway due to weight.

> If customer chooses not to be on site for delivery, the delivering material will be checked and photographed by the delivery driver. Any missing materials from order must be declared within 24 hours of initial delivery by customer.

> Exact time of deliveries cannot be confirmed due to any unforeseen variables that may arise hindering traffic, and/or plant production issues.

> Green Tree Metals needs to be notified 48 hours ahead of delivery for any changes in delivery date from customer. This allows adequate time to reschedule material delivery.
